This tutorial was written by me, March 16, 2014.  Any resemblance to any other tutorial is purely coincidental.  Please do not claim it as your own.

Ok let's begin.
Open a new canvas 650x650
Copy and paste Elems 50 Frame, and resize as needed.  Click inside with the magic wand and Modify, Expand by 10.  Copy and paste Paper 7, resize as needed and then invert and hit delete on keyboard.  Make sure the paper is under your frame.  While still selected, copy and paste your close up tube of choice, resize as needed and place where you want her. Duplicate close up tube and on the top tube layer Adjust Gaussian Blur 10.  Then change the blend mode to Hard Light. Then on the bottom tube layer change the blend mode to Luminance Legacy.  Dropshadow your frame and tube.  Make sure your tube and paper are under the frame.  On the very bottom layer copy and paste Paper 5 and apply your mask.  Merge group.

Copy and paste your main tube of choice and resize as needed and place where desired.  Now you can place whatever elements you would like, resize as needed and place where desired.  Dropshadow your elements and main tube.  Here are the elements I used:
